  • A list of medications you are currently taking (including dosage and times) and any past surgical procedures.
  • Your medical insurance card(s)
  • Your picture ID.
  • Any referral slips and x-rays/test results if applicable for an existing condition.
  • Your medical history.

Insurance

Insurance companies and their policies are known to vary greatly in regards to co-pays and what they cover.  Please check with your insurance company to determine what they require in order to cover your appointment with us, or our office can gladly determine that for you before we schedule you in.

 

Please help us to ensure that your visit does not cost you more than necessary by knowing what your insurance companies policies are.

Office Visits:  Full payment, copays, and deductibles are due at the time of your appointment.

 

Charges: Every effort is made to accurately estimate our charges through you insurance company, but please be aware that this will only be an estimate.  The final determination of payment will lie with your insurance carrier.

 

Caveat:  The fees discussed by us with you represent the fees for the services provided by the Moore County Family Health Clinic only.  Hospital, laboratory, imaging, or other physician services will not be included in this estimate.
